Im so stupid... Death Skull?!?!

I feel like a complete idiot saying this but where can i find death skull? I found it once and i didn't save the game.

So if someone could please give me a straight up answer I would very much appreciate it.

forgot ma pass so i spammed it now i'm locked out for 15 min lol

I'm pretty sure its a drop from the evil thingo on crack that looks like a warden around the middle of the map, theres a quest line you gotta finish to get up to killing her though, ramza helps out, it isn't really that hard xD though her blind is annoying =(

+ Higher Difficulty = Better Chance
